FORWARD In March 2001, the National Institutes of Health issued the following warning: "The number of Web sites offering health-related resources grows every day. Many sites provide valuable information, while others may have information that is unreliable or misleading."1 Furthermore, because of the rapid increase in Internet-based information, many hours can be wasted searching, selecting, and printing. Since only the smallest fraction of information dealing with arm pain is indexed in search engines, such as www.google.com or others, a non-systematic approach to Internet research can be not only time consuming, but also incomplete. This book was created for medical professionals, students, and members of the general public who want to know as much as possible about arm pain, using the most advanced research tools available and spending the least amount of time doing so. In addition to offering a structured and comprehensive bibliography, the pages that follow will tell you where and how to find reliable information covering virtually all topics related to arm pain, from the essentials to the most advanced areas of research. Public, academic, government, and peer-reviewed research studies are emphasized. Various abstracts are reproduced to give you some of the latest official information available to date on arm pain. CHAPTER 1. STUDIES ON ARM PAIN Overview In this chapter, we will show you how to locate peer-reviewed references and studies on arm pain.
The Combined Health Information Database The Combined Health Information Database summarizes studies across numerous federal agencies. To limit your investigation to research studies and arm pain, you will need to use the advanced search options. First, go to http://chid.nih.gov/index.html. From there, select the “Detailed Search” option (or go directly to that page with the following hyperlink: http://chid.nih.gov/detail/detail.html). The trick in extracting studies is found in the drop boxes at the bottom of the search page where “You may refine your search by.” Select the dates and language you prefer, and the format option “Journal Article.” At the top of the search form, select the number of records you would like to see (we recommend 100) and check the box to display “whole records.” We recommend that you type “arm pain” (or synonyms) into the “For these words:” box. Consider using the option “anywhere in record” to make your search as broad as possible. If you want to limit the search to only a particular field, such as the title of the journal, then select this option in the “Search in these fields” drop box. The following is what you can expect from this type of search: •
Nonoperative Treatment of Neck and Arm Pain Source: Current Opinion in Orthopedics. 10(2): 122-130. April 1999. Summary: This journal article provides health professionals with information on the nonoperative treatment of neck and arm pain resulting from indirect trauma. Patients who have neck and arm pain must be evaluated and treated comprehensively. The first step in treatment involves determining the most likely pain generator. Musculoskeletal structures that may potentially be painful include bone, muscles, ligaments, zygapophysial joint, and intervertebral disks. Neural structures that may mediate pain include the dorsal root ganglia and nerve roots. The goals of treating neck pain are to decrease pain, restore motion, and improve strength and function. Nonsurgical approaches are used to treat cervical sprains and strains. Cervical radiculopathy can usually be treated successfully without surgery. Analgesics are usually prescribed in

acute sprains, strains, and disk herniations. Nonnarcotic analgesics commonly prescribed include acetaminophen, aspirin, and nonsteroidal antiinflammatory drugs (NSAIDs). Neck pain caused from repetitive strain is treated by correcting the underlying mechanics causing the strain. Routine prescription of muscle relaxants for treating neck pain is not recommended. Neuropathic pain may be treated with NSAIDs, antidepressants that block norepinephrine, cortisone, and anticonvulsants. Tricyclic antidepressants may used to treat patients who have fibromyalgia with and with clinical depression. Soft cervical collars have not been shown to be effective in the treatment of acute neck pain associated with motor vehicle accidents; however, they are commonly used in the acute period after cervical disk herniation to minimize motion and nerve root irritation. Other nonsurgical approaches to treating neck pain include active or passive range of motion exercises; stretching exercises; physical methods such as heat, electrical stimulation, and ultrasound; and cervical nerve root or epidural steroid injections. In addition, workstation ergonomics should be addressed. Pain is often accompanied by depression, so clinical depression should be identified and aggressively treated. Chronic pain management techniques should be explored by patients who do not respond to conservative interventions. 1 figure, 2 tables, and 78 references.
Federally Funded Research on Arm Pain The U.S. Government supports a variety of research studies relating to arm pain. These studies are tracked by the Office of Extramural Research at the National Institutes of Health.2 CRISP (Computerized Retrieval of Information on Scientific Projects) is a searchable database of federally funded biomedical research projects conducted at universities, hospitals, and other institutions. Search the CRISP Web site at http://crisp.cit.nih.gov/crisp/crisp_query.generate_screen. You will have the option to perform targeted searches by various criteria, including geography, date, and topics related to arm pain. For most of the studies, the agencies reporting into CRISP provide summaries or abstracts. As opposed to clinical trial research using patients, many federally funded studies use animals or simulated models to explore arm pain. Project Title: FORCE AND MEASUREMENT DEVICE - PHASE II Principal Investigator & Institution: Cooper, Rory A.; Director; Three Rivers Holdings, Llc 1826 W Broadway Rd, Ste 43 Mesa, Az 85202 Timing: Fiscal Year 2002; Project Start 15-SEP-2000; Project End 30-APR-2004 Summary: Because of the repetitive trauma to the upper extremities that results from wheelchair propulsion, the prevalence of pain and injury in manual wheelchair users is alarmingly high. By understanding the mechanisms in the development of arm pain, we can begin to prevent the pain. The SMART(Wheel) is a tool that enables direct measurement of the forces and torques applied by wheelchair users to the pushrim. This capability allows for investigations into the cause of injuries, and can lead to specific

recommendations to improve efficiency and reduce the risk of injury. The specific aims in Phase II of this research are to: 1) Identify additional characteristics in a force/moment measure system (e.g., display content and layout) that are desired by clinicians; 2) Further refine the design of the Clinical SMART(Wheel) (both hardware and software components) to maximize its functionality for commercial use; 3) Conduct clinical testing to verify that the performance of the newly designed "Clinical SMART(Wheel)" is useful clinically; 4) Conduct off-site trials with manual wheelchair users to demonstrate the usability and effectiveness of the Clinical SMART(Wheel) in clinical or other environments. The culmination of this research will be a commercially viable product available to research laboratories, seating/wheelchair clinics, wheelchair and accessory manufacturers, ADA consultants, and manufacturers of accessible surfaces (e.g. playgrounds, parks, etc.). PROPOSED COMMERCIAL APPLICATION: The SMART(Wheel) will be a valuable tool for rehabilitation research labs, seating/wheelchair clinics, and wheelchair manufacturers. Many have already purchased SMART(Wheel)s, even when earlier versions of the SMART(Wheel) were less user-friendly. Because the SMART(Wheel) also facilitates the evaluation of the accessibility of surfaces, it will also be an important tool for ADA consultants, architects, regional planners, and manufacturers of accessible surfaces (e.g. playgrounds, parks, etc.). Project Title: SEX AND MENSTRUAL CYCLE EFFECTS ON PAIN AND ANALGESIA Principal Investigator & Institution: Fillingim, Roger B.; Associate Professor; Operative Dentistry; University of Florida Gainesville, Fl 32611 Timing: Fiscal Year 2002; Project Start 01-SEP-2000; Project End 31-MAR-2004 Summary: The influence of sex and gender on pain perception and pain modulatory systems is a research topic of substantial basic science and clinical import. Studies in nonhuman animals and humans indicate greater sensitivity to experimental pain among females. Sex differences in opioid analgesia have also been reported, but are not well characterized in humans. The mechanisms underlying these sex-related differences in pain modulator have not been determined; although, considerable evidence from animals suggests that baseline pain sensitivity and opioid-mediated analgesia vary across the estrous cycle. However, among humans, findings relating pain sensitivity to menstrual cycle phase are inconsistent, and the effects of menstrual cycle phase an opioid analgesia have not been investigated. The studies proposed in this application are designed to further elucidate the nature and mechanisms of sex- related differences in pain modulation by investigating baseline pain sensitivity and opioid analgesia as a function of sex, menstrual cycle phase and oral contraceptive use. Two clinically relevant laboratory pain induction procedures will be used in these studies: 1) temporal summation of thermal pain, and 2) ischemic arm pain. In the proposed study, responses to temporal summation and isochemic procedures will be obtained in females and males before and after administration of morphine, pentazocine and placebo (saline). In addition, two groups of females will be studies: females not taking oral contraceptives (OC) and females taking OC. Both groups of females will participate in each drug condition once during the follicular and once during the luteal phase of the menstrual cycle, and males will participate at similar time intervals. It is hypothesized that: 1) females, regardless of OC status, will exhibit greater baseline pain sensitivity than males, 2) females not taking OC will be more pain sensitive during the luteal compared to the follicular phase of the menstrual cycle, while females taking OC will show no

change in pain responses across cycle phase; 3) males will exhibit greater morphine analgesia than females, while females will exhibit greater analgesia to pentazocine than males, and 4) females not taking OC will exhibit less opioid analgesia during the luteal versus the follicular phase of the menstrual cycle, while no menstrual cycle effects on analgesia will emerge for females taking OC. Project Title: SHAM DEVICE, PILL PLACEBO OR TREATMENT FOR ARM PAIN Principal Investigator & Institution: Kaptchuk, Ted J.; Assistant Professor of Medicine; Medicine; Harvard University (Medical School) Medical School Campus Boston, Ma 02115 Timing: Fiscal Year 2002; Project Start 01-SEP-2000; Project End 30-JUN-2004 Summary: (adapted from investigator's abstract): This application is based on observations that the magnitude of the placebo effect produced by a device is greater than that produced by a pill. If this is generally true, there are significant implications for the design of clinical trials involving medical devices and for our understanding of the role of the placebo effect in randomized controlled trials (RCT). This is a two phase study. In the first phase, the investigators will estimate the relative magnitudes of placebo effects associated with a sham acupuncture procedure versus an inactive oral treatment in patients with persistent upper extremity pain secondary to repetitive strain injury (RSI) most notably, carpal tunnel syndrome (CTS). In the second phase, parallel RCTs of traditional Chinese acupuncture (TCA) and low-dose amitriptyline (AMI) versus their respective placebo treatments will be conducted. In Phase I, 240 patients with RSI will be randomly assigned to receive a placebo device (a recently validated sham acupuncture device) or a placebo pill (dummy AMI). The primary hypothesis in Phase I is that patients will respond better to the sham device than the placebo pill. Phase II will see patients in each arm of Phase I randomly assigned to active or inactive treatment. Patients from the sham acupuncture arm of Phase I will receive either TCA or continue to receive the sham version. Patients in the placebo pill arm of Phase I will receive either AMI or continue receiving the placebo pill. Outcomes in these studies will include assessments of the nature and severity of upper extremity pain, function, grip and pinch strength, and mood. Project Title: STRESS, ADRENERGIC AND INFLAMMATORY FACTORS IN 4 DISORDERS Principal Investigator & Institution: Light, Kathleen C.; University of North Carolina Chapel Hill Aob 104 Airport Drive Cb#1350 Chapel Hill, Nc 27599 Timing: Fiscal Year 2002 Summary: Chronic fatigue syndrome (CFS), Temporomandibular Disorder (TMD) and Fibromyalgia (FM) are common chronic disabling disorders whose pathogenesis and treatment are not well understood, but which share four characteristics: sensitivity to life stress, signs of pain system dysregulation, psychological distress and negative affect, and possible alteration of inflammatory mediators. The focus of the present investigation is to compare 40 patients meeting accepted diagnostic criteria for each of these disorders with 40 age- and gender-matched healthy controls and with 40 patients diagnosed criteria for each of these disorders with 40 age- and gender-matched healthy controls and with 40 patients diagnosed with Rheumatoid Arthritis (RA), the prototypical chronic inflammatory disorder. To determine whether there is evidence of

dysregulation of autonomic (particularly beta-adrenergic function, hypothalamicpituitary adrenocortical function (HPA), endogenous opioids, and inflammatory cytokine responses, these interacting physiological systems will be assessed during baseline and in response to two standardized stressors, a speech about interpersonal conflict and tourniquet-induced ischemic arm pain. Prior research has confirmed betaadrenergic mediation of stress-induced changes in immune parameters. Thus, each subject will be studied twice, once after placebo and once after acute pretreatment with the non-selective beta-receptor antagonist, propranolol, to confirm the hypothesized involvement of beta-receptor activity in the dysregulated responses of the CFS, TMD and FM groups. In a second study, these same patients will be recruited to enter a placebo-controlled, double-blind cross-over treatment trial (6 weeks) of propranolol's potential benefits in normalizing responses to lab stressors and real life demands, in decreasing pain hypersensitivity, and improving somatic and psychological symptoms. A novel aspect of these studies will be their focus on the relationships between HPA axis function, autonomic function and effects upon IL6, IL1beta, and TNFalpha, the cytokines forming the central cascade in initiation of the inflammatory response. This investigation will provide important and needed assessment of basic physiological alterations, as well as more concrete tests of the contribution of stress exposure, in CFS, TMD and FM patients. Further, by clarifying the hypothesized role of beta-adrenergic activity and benefits of beta-blockade, it also provides a starting point for research on more effective medical treatment in disorders which have been medically difficult to manage. Coffee Break: Tutorials for Biologists12 Coffee Break is a general healthcare site that takes a scientific view of the news and covers recent breakthroughs in biology that may one day assist physicians in developing treatments. Although most cervical disk problems improve with conservative care, many patients find relief from pain and weakness only after cervical disk surgery. The booklet discusses the anatomy of the cervical spine, its importance, and conditions that contribute to damaging the structures of the cervical spine. Evaluation by the doctor may include a medical history, physical exam, and diagnostic tests. Based on the exam and test results, the doctor may first prescribe nonsurgical care. Nonsurgical treatments include using medications to relieve symptoms, performing physical therapy to restore movement and strength, and using exercises to prevent further injury. Summary: This journal article uses a question and answer format to provide people who have cervical spondylotic myelopathy (CSM) with information on the cause, symptoms, diagnosis, and treatment of this spinal cord disorder. CSM, which most often affects older adults, is a compression of the spinal cord at the neck. Normal aging and arthritis cause changes in the bones, discs, and ligaments of the spine that put pressure on the spinal cord. Symptoms include neck stiffness, arm pain, numbness in the hands, and weakness of the arms and legs. Diagnosis is based on a physical examination and a magnetic resonance imaging scan. Mild cases of CSM can be treated with braces or traction; however, the long term effectiveness of these treatments is unknown. Surgery to reduce the compression of the spinal cord may be successful in some people. Medications may relieve the pain of CSM but do not help the underlying cause of the condition. Finding Associations There are several Internet directories that provide lists of medical associations with information on or resources relating to arm pain. By consulting all of associations listed in this chapter, you will have nearly exhausted all sources for patient associations concerned with arm pain. The National Health Information Center (NHIC) The National Health Information Center (NHIC) offers a free referral service to help people find organizations that provide information about arm pain. For more information, see the NHIC’s Web site at http://www.health.gov/NHIC/ or contact an information specialist by calling 1-800-336-4797. Directory of Health Organizations The Directory of Health Organizations, provided by the National Library of Medicine Specialized Information Services, is a comprehensive source of information on associations. The Directory of Health Organizations database can be accessed via the Internet at http://www.sis.nlm.nih.gov/Dir/DirMain.html. It is composed of two parts: DIRLINE and Health Hotlines. The DIRLINE database comprises some 10,000 records of organizations, research centers, and government institutes and associations that primarily focus on health and biomedicine. To access DIRLINE directly, go to the following Web site: http://dirline.nlm.nih.gov/. Simply type in “arm pain” (or a synonym), and you will receive information on all relevant organizations listed in the database. Health Hotlines directs you to toll-free numbers to over 300 organizations. You can access this database directly at http://www.sis.nlm.nih.gov/hotlines/. On this page, you are given the option to search by keyword or by browsing the subject list. When you have received your search results, click on the name of the organization for its description and contact information. The Combined Health Information Database Another comprehensive source of information on healthcare associations is the Combined Health Information Database. Using the “Detailed Search” option, you will need to limit your search to “Organizations” and “arm pain”. Type the following hyperlink into your Web browser: http://chid.nih.gov/detail/detail.html. To find associations, use the drop boxes at the bottom of the search page where “You may refine your search by.” For publication date, select “All Years.” Then, select your preferred language and the format option “Organization Resource Sheet.” Type “arm pain” (or synonyms) into the “For these words:” box. You should check back periodically with this database since it is updated every three months.

APPENDIX C. FINDING MEDICAL LIBRARIES Overview In this Appendix, we show you how to quickly find a medical library in your area.
Preparation Your local public library and medical libraries have interlibrary loan programs with the National Library of Medicine (NLM), one of the largest medical collections in the world. According to the NLM, most of the literature in the general and historical collections of the National Library of Medicine is available on interlibrary loan to any library. If you would like to access NLM medical literature, then visit a library in your area that can request the publications for you.15
Finding a Local Medical Library The quickest method to locate medical libraries is to use the Internet-based directory published by the National Network of Libraries of Medicine (NN/LM). This network includes 4626 members and affiliates that provide many services to librarians, health professionals, and the public. To find a library in your area, simply visit http://nnlm.gov/members/adv.html or call 1-800-338-7657.

ARM PAIN DICTIONARY The definitions below are derived from official public sources, including the National Institutes of Health [NIH] and the European Union [EU]. Acetaminophen: Analgesic antipyretic derivative of acetanilide. It has weak antiinflammatory properties and is used as a common analgesic, but may cause liver, blood cell, and kidney damage. [NIH] Adenocarcinoma: A malignant epithelial tumor with a glandular organization. [NIH] Adrenal Medulla: The inner part of the adrenal gland; it synthesizes, stores and releases catecholamines. [NIH] Adrenergic: Activated by, characteristic of, or secreting epinephrine or substances with similar activity; the term is applied to those nerve fibres that liberate norepinephrine at a synapse when a nerve impulse passes, i.e., the sympathetic fibres. [EU] Agonist: In anatomy, a prime mover. In pharmacology, a drug that has affinity for and stimulates physiologic activity at cell receptors normally stimulated by naturally occurring substances. [EU] Algorithms: A procedure consisting of a sequence of algebraic formulas and/or logical steps to calculate or determine a given task. [NIH] Alkaloid: A member of a large group of chemicals that are made by plants and have nitrogen in them. Some alkaloids have been shown to work against cancer. [NIH] Allograft: An organ or tissue transplant between two humans. [NIH] Aloe: A genus of the family Liliaceae containing anthraquinone glycosides such as aloinemodin or aloe-emodin (emodin). [NIH] Alpha-1: A protein with the property of inactivating proteolytic enzymes such as leucocyte collagenase and elastase. [NIH] Alternative medicine: Practices not generally recognized by the medical community as standard or conventional medical approaches and used instead of standard treatments. Alternative medicine includes the taking of dietary supplements, megadose vitamins, and herbal preparations; the drinking of special teas; and practices such as massage therapy, magnet therapy, spiritual healing, and meditation. [NIH] Amitriptyline: Tricyclic antidepressant with anticholinergic and sedative properties. It appears to prevent the re-uptake of norepinephrine and serotonin at nerve terminals, thus potentiating the action of these neurotransmitters. Amitriptyline also appears to antaganize cholinergic and alpha-1 adrenergic responses to bioactive amines. [NIH] Anaesthesia: Loss of feeling or sensation. Although the term is used for loss of tactile sensibility, or of any of the other senses, it is applied especially to loss of the sensation of pain, as it is induced to permit performance of surgery or other painful procedures. [EU] Analgesic: An agent that alleviates pain without causing loss of consciousness. [EU] Anatomical: Pertaining to anatomy, or to the structure of the organism. [EU] Anesthesia: A state characterized by loss of feeling or sensation. This depression of nerve function is usually the result of pharmacologic action and is induced to allow performance of surgery or other painful procedures. Angina Pectoris: The symptom of paroxysmal pain consequent to myocardial ischemia usually of distinctive character, location and radiation, and provoked by a transient stressful situation during which the oxygen requirements of the myocardium exceed the capacity of the coronary circulation to supply it. [NIH] Antibiotic: A drug used to treat infections caused by bacteria and other microorganisms. [NIH]
Antibody: A type of protein made by certain white blood cells in response to a foreign substance (antigen). Each antibody can bind to only a specific antigen. The purpose of this binding is to help destroy the antigen. Antibodies can work in several ways, depending on the nature of the antigen. Some antibodies destroy antigens directly. Others make it easier for white blood cells to destroy the antigen. [NIH] Anticholinergic: An agent that blocks the parasympathetic nerves. Called also parasympatholytic. [EU] Anticonvulsants: Drugs used to prevent seizures or reduce their severity. [NIH] Antidepressant: A drug used to treat depression. [NIH] Antidote: A remedy for counteracting a poison. [EU] Antigen: Any substance which is capable, under appropriate conditions, of inducing a specific immune response and of reacting with the products of that response, that is, with specific antibody or specifically sensitized T-lymphocytes, or both. Antigens may be soluble substances, such as toxins and foreign proteins, or particulate, such as bacteria and tissue cells; however, only the portion of the protein or polysaccharide molecule known as the antigenic determinant (q.v.) combines with antibody or a specific receptor on a lymphocyte. Abbreviated Ag. [EU] Anti-inflammatory: Having to do with reducing inflammation. [NIH] Anti-Inflammatory Agents: Substances that reduce or suppress inflammation. [NIH] Antimetabolite: A chemical that is very similar to one required in a normal biochemical reaction in cells. Antimetabolites can stop or slow down the reaction. [NIH] Antineoplastic: Inhibiting or preventing the development of neoplasms, checking the maturation and proliferation of malignant cells. [EU] Antipyretic: An agent that relieves or reduces fever. Called also antifebrile, antithermic and febrifuge. [EU] Anxiety: Persistent feeling of dread, apprehension, and impending disaster. [NIH] Aorta: The main trunk of the systemic arteries. [NIH] Arterial: Pertaining to an artery or to the arteries. [EU] Arteries: The vessels carrying blood away from the heart. [NIH] Artery: Vessel-carrying blood from the heart to various parts of the body. [NIH] Articular: Of or pertaining to a joint. [EU] Aspirin: A drug that reduces pain, fever, inflammation, and blood clotting. Aspirin belongs to the family of drugs called nonsteroidal anti-inflammatory agents. It is also being studied in cancer prevention. [NIH] Asymptomatic: Having no signs or symptoms of disease. [NIH] Axillary: Pertaining to the armpit area, including the lymph nodes that are located there. [NIH]
Axillary Vein: The venous trunk of the upper limb; a continuation of the basilar and brachial veins running from the lower border of the teres major muscle to the outer border

of the first rib where it becomes the subclavian vein. [NIH] Base: In chemistry, the nonacid part of a salt; a substance that combines with acids to form salts; a substance that dissociates to give hydroxide ions in aqueous solutions; a substance whose molecule or ion can combine with a proton (hydrogen ion); a substance capable of donating a pair of electrons (to an acid) for the formation of a coordinate covalent bond. [EU] Basophils: Granular leukocytes characterized by a relatively pale-staining, lobate nucleus and cytoplasm containing coarse dark-staining granules of variable size and stainable by basic dyes. [NIH] Benign: Not cancerous; does not invade nearby tissue or spread to other parts of the body. [NIH]
Bile: An emulsifying agent produced in the liver and secreted into the duodenum. Its composition includes bile acids and salts, cholesterol, and electrolytes. It aids digestion of fats in the duodenum. [NIH] Biotechnology: Body of knowledge related to the use of organisms, cells or cell-derived constituents for the purpose of developing products which are technically, scientifically and clinically useful. Alteration of biologic function at the molecular level (i.e., genetic engineering) is a central focus; laboratory methods used include transfection and cloning technologies, sequence and structure analysis algorithms, computer databases, and gene and protein structure function analysis and prediction. [NIH] Blood vessel: A tube in the body through which blood circulates. Blood vessels include a network of arteries, arterioles, capillaries, venules, and veins. [NIH] Bone Marrow: The soft tissue filling the cavities of bones. Bone marrow exists in two types, yellow and red. Yellow marrow is found in the large cavities of large bones and consists mostly of fat cells and a few primitive blood cells. Red marrow is a hematopoietic tissue and is the site of production of erythrocytes and granular leukocytes. Bone marrow is made up of a framework of connective tissue containing branching fibers with the frame being filled with marrow cells. Treatment allocations using coin flips, odd-even numbers, patient social security numbers, days of the week, medical record numbers, or other such pseudo- or quasi-random processes, are not truly randomized and trials employing any of these techniques for patient assignment are designated simply controlled clinical trials. [NIH] Receptor: A molecule inside or on the surface of a cell that binds to a specific substance and causes a specific physiologic effect in the cell.
