Fluorescein angiography is an indispensable tool in modern ophthalmology. It is based on the behavior of intravenously administered sodium fluorescein in the tissues of the ocular fundus. The phenomena observed provide the ophthalmologist with valuable diagnostic information. This book describes the morphological characteristics and structural changes underlying these phenomena.

Die Fluoreszeinangiographie ist ein unverzichtbares Werkzeug der modernen Augenheilkunde. Sie basiert auf dem Verhalten intravenös applizierten Natriumfluoreszeins in den Geweben des Augenhintergrunds. Die dabei beobachteten Phänomene liefern dem Augenarzt wertvolle diagnostische Hinweise. Das vorliegende Buch beschreibt die morphologischen Merkmale und strukturellen Veränderungen, die diesen Phänomenen zugrunde liegen.

La angiograf´ıa con fluoresce´ına es una herramienta indispensable en la oftalmolog´ıa moderna. Esta se basa en el comportamiento de la fluoresce´ına sodica ´ administrada intravenosamente en tejidos del fondo ocular. Los fenomenos ´ observados proveen al oftalmologo ´ de una informacion ´ diagnostica ´ de gran valor. Este libro describe las caracter´ısticas morfologicas ´ y cambios estructurales que soportan estos fenomenos. ´

Fluorescein angiography is a remarkable in vivo technique, because it is able to resolve the minute capillaries in the neighborhood of the foveal avascular zone which measure only 3.5 µm in diameter (top). This is half the diameter of red blood cells. Because of their small diameter, the capillaries in the vicinity of the foveal avascular zone cannot be perfused with whole blood, since the red blood cells have to change their shape in order to pass, thus requiring much more space (bottom).

Die Fluoreszeinangiographie ist eine bemerkenswerte In-vivo-Untersuchungsmethode, denn sie ist in der Lage, die winzigen Kapillaren in der Nachbarschaft der fovealen avaskulären Zone, die einen Durchmesser von nur 3,5 µm haben, darzustellen (oben). Dies ist der halbe Durchmesser eines Erythrozyten. Aufgrund ihres geringen Durchmessers können die Kapillaren in der Nähe der fovealen avaskulären Zone nicht von Vollblut durchströmt werden. La angiograf´ıa con fluoresce´ına es una tecnica ´ in vivo extraordinaria, debido a que es capaz de resolver capilares diminutos en la vecindad de la zona foveal avascular los cuales miden solamente 3.5 micras de diámetro (superior). Esta es la mitad del diámetro de los glóbulos rojos. Debido al pequeño diámetro de los capilares en la vecindad de la zona foveal avascular la perfusión no se realiza con todos los elementos de la sangre, los glóbulos rojos tienen que deformarse para circular, requiriendo as´ı más espacio (inferior).

The cell concentration in the retinal vessels further away from the foveal avascular zone is increased. The bulk of surplus cells is most likely transported to the venous side by wide vascular channels, so-called shunt capillaries, in the far periphery of the retina. These capillaries form long (top) or short (middle) loops in the immediate vicinity of the ora serrata. Their lumen is up to 20 µm wide (bottom).

Die Zellkonzentration in den Retinagefäßen in weiterer Entfernung von der fovealen avaskulären Zone steigt an. Die Mehrzahl dieser Zellen wird offenbar über weite Gefäßkanäle in der äußersten Peripherie, sog. Shunt-Kapillaren zur venösen Seite transportiert. Diese Kapillaren bilden lange (oben) oder kurze (Mitte) Schlingen in unmittelbarer Nachbarschaft der Ora serrata. Ihr Lumen ist bis zu 20 µm weit (unten).

La concentracion ´ de celulas ´ en los vasos de la retina mas ´ alla´ de la zona foveal avascular se incrementa. El volumen excedente de celulas ´ es muy probable que sea transportado al lado venoso por canales vasculares anchos, llamados capilares “shunt”, en la extrema periferia de la retina. The neurosensory retina is transparent. For this reason the so-called background fluorescence, i.e. the staining of the underlying choroids, becomes visible. It appears as a diffuse gray haze behind much brighter stained retinal vessels.

A focal absence of hemidesmosomes allows the retinal pigment epithelium to split away from Bruch’s membrane so that fluorescein-stained fluid can accumulate between them. In young adults this condition is also called central serous chorioretinopathy type II.

The migration of fluorescein follows the rules of diffusion. Diffusion is the movement of molecules in liquids or gases from regions of high concentration to regions of low concentration. Like all other tissues the retina and the choroid are composed of cells and intercellular spaces. In general, intercellular spaces represent the regular pathways for the diffusion of fluorescein, while the cells form a barrier against the free diffusion of fluorescein; however, there are two important exceptions to this rule. One exception is the so-called capillary pores; the other is the so-called zonulae occludentes. red bars = diffusion obstacles

Opening of zonulae occludentes is typically the case in inflammatory conditions. In retinal vasculitis fluorescein leaks through the open intercellular spaces of the endothelium. It stains the walls and the vicinity of the diseased vessel and gives rise to the erroneous term “perivasculitis”.

In summary, the layer of zonulae occludentes of the retinal pigment epithelium thus represents the blood-retinal barrier for substances circulating through the pore-bearing, leaky choroidal vessels, while for the retinal vessels the barrier is located in the vascular wall itself, as explained previously (p. 68 ff.).

The diffusion of material against the stream is illustrated by the small rim of migrating fluorescein that stains the periphery of photocoagulation scars in the region where the choriocapillaris has remained open. The retinochoroidal fluid movement across breaks in the pigment epithelial barrier explains why photocoagulation scars drain fluid accumulations in, or under, the neurosensory retina into the choroid.

A very small focal breakdown of the retinal pigmenthelial diffusion barrier occurs in the so-called type I of central serous chorioretinopathy (CSR). Through this leakage point fluid from the choroid can cross the pigment epithelium and accumulate under the neurosensory retina. As a typical sign of leakage the leakage point seems to grow. Since simple breakdown of the pigment epithelial diffusion barrier would lead to fluid movement in retinochoroidal direction (p. 85), the massive fluid movement of CSR in chorioretinal direction must be the result of a more complex process.

In about 6% of the cases of central serous chorioretinopathy the fluorescein passing through the pigment epithelial leakage point moves upwards and may spread in an umbrella-like fashion. This finding, the so-called smoke-stack phenomenon, is said to be related to differences in either the temperature or the specific weight between the leaking fluid and the liquid that has already accumulated under the neurosensory retina.

Bei etwa 6% der Fälle von Chorioretinopathia contralis serosa steigt das Fluoreszein, das sich durch den pigmentepithelialen Quellpunkt bewegt, nach oben auf und breitet sich schirmartig aus. Dieser als „Rauchfahnenphänomen“ bezeichneter Befund soll mit dem Unterschied der Temperatur oder des spezifischen Gewichts der leckenden Flüssigkeit und der Flüssigkeit, die sich bereits unter der Neuroretina angesammelt hat, zusammenhängen.

En cerca del 6% de los casos de Coriorretinopat´ıa serosa central el paso de fluoresce´ına a traves ´ del punto de fuga del epitelio pigmentado se mueve hacia arriba y puede dispersarse en una forma parecida a una sombrilla. Este hallazgo, llamado “fenómeno en humo de chimenea” parece estar relacionado a diferencias en la temperatura o el peso espec´ıfico entre el l´ıquido que fuga y el l´ıquido que ya se ha acumulado debajo de la retina neurosensorial.

Normal retinal pigment epithelium actively absorbs fluid in a retinochoroidal direction. This is accomplished by the movement of certain ions. It is conceivable that, under the influence of an as yet undefined damaging process in the pigment epithelium or the underlying choroid, a minute group of RPE cells, possibly even a single cell, reverses its function and secretes large amounts of ions in chorioretinal direction, i.e. into the subneuroretinal space, thus attracting choroidal fluid into this area. Initially, the fluid movement certainly occurs transcellularly. The flow is so strong, however, that it possibly disrupts the pigment epithelium in the involved area and sometimes even detaches the base of the surrounding RPE cells from Bruch’s membrane. Since the defective area of pigment epithelium is very small, only a tiny leakage point is visible during the earliest phase of fluorescein angiography.

Es ist denkbar, dass unter dem Einfluss eines bisher nicht bekannten krankhaften Prozesses im Pigmentepithel oder in der benachbarten Aderhaut eine winzige Gruppe von Pigmentepithelzellen, oder sogar nur eine einzige Zelle ihre Funktion umkehren und große Mengen von Ionen in die falsche Richtung, d. h. unter die Neuroretina sezernieren und so Aderhautflüssigkeit in dieses Gebiet ziehen. Anfänglich geschieht dies sicherlich transzellulär. Der Flüssigkeitsstrom ist aber so stark, dass er möglicherweise das Pigmentepithel in dem betroffenen Gebiet aufreißt und bisweilen sogar die umgebenden Pigmentepithelzellen von der Bruch’schen Membran abhebt. Da der Defekt sehr klein ist, ist zuerst nur ein winziger Quellpunkt sichtbar.

El epitelio pigmentado de la retina normal absorbe l´ıquido activamente en una dirección retino-coroidea. Esto se logra debido al movimiento de ciertos iones. Es concebible que, bajo la influencia de un proceso dañino aún indefinido en el epitelio pigmentado o en la coroides subyacente, un pequeño grupo de células del epitelio pigmentado de la retina, posiblemente incluso una sola célula, revierta su función y secrete grandes cantidades de iones en dirección corio-retiniana, i.e., dentro del espacio subneurorretiniano, atrayendo as´ı al l´ıquido coroideo a esta a´ rea. Inicialmente, el movimiento de l´ıquido ocurre ciertamente de manera transcelular. Sin embargo el flujo es tan fuerte, que posiblemente rompe el epitelio pigmentado en el a´rea involucrada y ocasionalmente incluso se desprenden la base de las células del EPR circundantes de la membrana de Bruch. Dado que el a´ rea defectuosa de epitelio pigmentado es muy pequeña, solo es visible un pequeño punto de fuga durante la fase más temprana de la angiograf´ıa con fluoresce´ına.

The subretinal blister does not leak fluorescein into the surrounding retina and from there into the vitreous. It is likely that the intercellular spaces surrounding the blister are sealed off by newly formed tight junctions as they occur in other retinal fluid accumulations. It is safe to conclude that the fluid that enters the blister through the leakage point is pumped back into the choroid by the pigment epithelial cells neighboring the leakage point. After healing and as a consequence of the stress, the cells involved show a lasting pigmentary disturbance in the entire area underlying the former neurosensory detachment.

Aus der subneuroretinalen Blase leckt kein Fluoreszein in die umgebende Netzhaut und von hier in den Glaskörper. Wahrscheinlich sind die Interzellularräume, die die Blase umgeben, durch neugebildete Zonulae occludentes, wie sie bei anderen retinalen Flüssigkeitsansammlungen vorkommen, verschlossen. Man kann daher folgern, dass die durch den Quellpunkt in die Blase gelangende Flüssigkeit durch die benachbarten Pigmentepithelzellen in die Aderhaut zurückgepumpt wird. Als Stressfolge bleiben nach Abheilung in dem gesamten Gebiet der ehemaligen Blase Pigmentstörungen zurück.

La ves´ıcula subretiniana no fuga fluoresce´ına hacia la retina circundante y de ah´ı al v´ıtreo. Es probable que los espacios intercelulares que rodean la ves´ıcula sean sellados por uniones intercelulares estrechas de reciente formación, as´ı como ocurre en otras acumulaciones de l´ıquido retiniano. Es seguro concluir que el l´ıquido que entra a la ves´ıcula a través del punto de fuga es bombeado de regreso a la coroides por las células de epitelio pigmentado cercanas al punto de fuga. Después de recuperarse y como consecuencia del estrés, las células involucradas muestran un disturbio pigmentario duradero en toda el a´ rea subyacente al antiguo desprendimiento neurosensorial.

Type III central serous chorioretinopathy is a combination of the two previous types comprising both a pigment epithelial detachment (p. 60/61) and a neurosensory detachment (p. 108/109 ff.) with a leakage point located within (A) or outside (B) the area of pigment epithelial detachment. The angiogram of such a condition reveals only pooling and no leakage of dye in the area of pigment epithelial detachment and the stained area does not change its size (asterisk), whereas the leakage point in the pigment epithelium seems to grow (arrow).

Detachment of the retinal pigment epithelium (p. 60/61) and/or the neurosensory retina (p. 108/109 ff.) can be caused also by choroidal neovascularizations which are found in a variety of conditions. When the proliferated vessels lie below the retinal pigment epithelium (a) their visibility depends on the amount of pigment granules in the overlying retinal pigment epithelium; however, when the newly formed vessels extend through the pigment epithelium into the subneuroretinal space, (b) they are visible due to the transparency of the neurosensory retina. The dark ring surrounding the proliferated vessels in the case presented on the opposite page is created by blood that blocks underlying fluorescence.
